Changing My Account Details

The forms listed below can be used to change information about your account. Simply download, print, fill in and send us the forms below:

• I want to change my investment portfolio (PDF 440KB)
• I want to change my personal details (PDF 73KB)
• I want to change my contribution details (PDF 27KB)
• Changes to employment (one participating in SSRSS) (PDF 29KB)

Requesting A Withdrawal

To request your withdrawal simply download, print, fill in and send us the relevant form below:

• I want to make a withdrawal (PDF 82KB)
• Fund Withdrawal Tax form - to be completed with all withdrawals (PDF 57KB)
• Declaration on leaving the State Sector. This form is to be used when withdrawing under the clause attaining aged 50 and leaving the State Sector (PDF 23KB)

What's My Monthly Member Fee?
