It’s easy to claim. If you’re an AXA Personal or Business Insurance customer you can begin our ‘fast and fair’ process right here…

You simply complete a claim form and provide some supporting information, we’ll do the rest. To get things started scroll down to the type of claim you want to make, and you can download a claim form right away:

  • Income Protection Claim
  • Trauma, Terminal Illness and Disablement Claims
  • Death Claim
  • Group Claim

To find out about your group insurance claim, please call your adviser or employer in the first instance.

For general enquiries about claims, you can contact our helpful Customer Service Team on 0800 106 652.

Make an Income Protection Claim

Make sure you ask your Doctor for:

  • Your Certificate of Medical Attendance
  • Any available medical information that will support your claim
  • A copy of your full medical history notes

You may also need to provide some other information:

  • Personal tax return
  • Bank deposit slip
  • Hospital discharge form - if applicable
  • Copies of any ACC documentation including your final assessment of weekly compensation - if applicable
  • If you are claiming for a stress/depression related disorder, you’ll need to provide the details of your diagnosis from a psychiatrist

If you are a wage or salary earner make sure you provide:

  • Wage slip
  • Copy of employment contact

If you are self-employed make sure you provide:

  • Your tax assessment
  • Copy of full business financial statements

Trauma Benefit Claim

Make sure you ask your Doctor for:

Terminal Illness Claim

  • Your Medical certificate completed
  • Any available medical information that will support your claim
  • A copy of your full medical history notes

Disablement Claim

  • Copy of your full medical history notes

Make a Death Claim 

To make a claim the following will need to be provided:

  • Registrar’s Certificate of Death
  • A Completed Payment Discharge Form
  • Medical history notes, coroner’s findings and accident reports (if applicable) will also need to be provided.

In some circumstances we may require proof of administration such as:

  • Letters of Administration
  • Copies of Probate
  • Administration Act Form

Once all the information has been collected, the Death Certificate, Discharge form and any other requirements should be forwarded to:

Our claims team will assess the claim, which may include assessing the Life Insured’s medical details against the form completed when the cover was first applied for.  We may also ask for other information from you or the claimant or The Estate before completing the process.
